Preventive Club:

• 2 Prophy visits per year: $29.99/month ($359.88)
• 3 Prophy visits per year: $39.99/month ($479.88)
• Includes:
  • Necessary cleanings
  • All necessary dental examinations
  • All necessary x-rays (CT Scan is not an x-ray)
  • 2 emergency evaluation visits, if needed
  • 2 fluoride treatments
  • 10% discount off any additional treatment
• Children: $24.99/month ($299.88)
  • Includes 2 cleanings per year plus the other items
  • ½ price preventive sealants
  • Annual oral development evaluation

Periodontal Preventive Club:

• 2 periodontal visits per year: $39.99/month ($479.88)
• 3 periodontal visits per year: $49.99/month ($599.88)
• 4 periodontal visits per year: $59.99/month ($719.88)
• Includes:
  • Necessary periodontal cleanings
  • All necessary examinations
  • All necessary x-rays (CT Scan is not an x-ray)
  • 2 emergency evaluation visits, if needed
  • 2 fluoride treatments
  • 10% discount off any additional treatment

Additional Information:

• All monthly dues must be set-up for automatic charge to a credit or debit card and email.
• There is a one-time credit card fee that will be charged with the first payment at every renewal date.
• No waiting periods! You are eligible for your first visit after the first monthly due is paid.
• You must qualify for one of the above plans. Your individual plan will be determined by your dental hygienist and may require an initial evaluation appointment.
• Your dental health will be re-evaluated each year to determine if you still qualify for your enrolled plan.
• Patients with active periodontal disease may be required to complete comprehensive periodontal therapy before qualifying for one of the above club plans.
• Prices will be determined at your initial examination.
• You are signing up for 12 months.
