Children:

• Includes:
  • All of the above
  • 1/2 price preventative sealants
  • Annual oral development evaluation

Periodontal Prevention Club:

• 2 periodontal visits per year: $39.99/ month
• 3 periodontal visits per year: $49.99/ month
• 4 periodontal visits per year: $59.99/ month
• Includes:
  • Necessary periodontal cleanings
  • All necessary examinations
  • All necessary x-rays
  • 2 emergency evaluation visits, if needed
  • 2 fluoride treatments
  • 10% discount off any additional treatment items

Additional Information:

• All monthly dues must be set-up for automatic charge to a credit or debit card.
• No waiting periods! You are eligible for your first visit after the first monthly due is paid.
• You must qualify for one of the above plans. Your individual plan will be determined by your dental hygienist and may require an initial evaluation appointment.
• Your dental health will be re-evaluated each year to determine if you still qualify for your enrolled plan.
• Patients with active periodontal disease may be required to complete comprehensive periodontal therapy before qualifying for one of the above club plans. Prices will be determined at your initial examination.
